Clinical Chart note copies

All relevant pre, and current clinical chart records including intake and registration forms, specific injury/accident questionnaires, diagnostic imaging or other investigative reports (i.e. nerve conduction) surgical/procedural reports, specialist consultation reports, AB forms, medical updates/progress reports, discharge summaries. These should be in MS Word or .pdf format and organized chronologically or in separate folders as the case may be.

What all clinicians preparing reports for clients involved in significant that continue to have ongoing complaints of pain should be aware of are the research supporting prognostic conclusions. A study published in the Journal of Pain Medicine in January 2014 involved an extensive review of relevant references which included several online databases, independent studies, textbooks, and other printed reference materials dating back to 1966. They concluded there was consistent evidence in all methods used that neuropathic pain is present in chronic LBP and soft-tissue syndromes.
